Voters must submit an Absentee Ballot Request Form (PDF) if voting before Election Day. The Official Absentee Ballot Request form prescribed by the Iowa Secretary of State’s office is preferred to request an absentee ballot.
Each request form must include:
The name and signature of the registered voter
The registered voter's date of birth
The address at which the voter is registered to vote
The registered voter's voter verification number
The name or date of the election for which the absentee ballot is requested
Such other information as may be necessary to determine the correct absentee ballot for the registered voter
Submit the Request
Submit your request for an absentee ballot 120 days or less before the election. Your County Auditor must receive the completed request to mail no later than 5 p.m., ten days before a general election and eleven days before any other election. You may vote in person at the Election Office until 5:00 pm on the Monday before the election.
Only one person may request a ballot on a request form. Two or more people cannot share a request form. You must also use a separate request form for each election for which you are requesting a ballot.
